  • Appeal for Maximum Timeframe

    This form is used to determine eligibility of receiving additional Federal Student Aid according to the timeframe requirement of the Satisfactory Academic Progress (SAP) Policy.

    • Submit your Appeal For Maximum Timeframe to Academic Advisement.
    • After its review, Academic Advisement will forward it to Financial Aid for evaluation and a final determination of eligibility.
    • You will be officially notified of the outcome in the my.maricopa.edu Student Center.
  • Addendum to Maximum Timeframe

    This form is used to make changes to an approved maximum time frame appeal. Changes to your appeal would include adding or removing a class, requesting to repeat a course, or swapping out courses. This form can only be submitted if you already have an approved maximum time frame on file.

    • You must explain in detail the extenuating circumstances surrounding your need to repeat the class(es) and attach documentation to support your appeal (i.e. medical claims/statement, police report, etc.).
    • Submit your completed addendum to Academic Advisement.
    • After its review, Academic Advisement will forward it to Financial Aid for evaluation and a final determination of eligibility.
    • You will be officially notified of the outcome in the my.maricopa.edu Student Center.
  • Appeal for Satisfactory Academic Progress

    Students have the right to submit an appeal to address why he or she was unable to meet the standards of the Rio Salado College Satisfactory Academic Progress (SAP) Policy. A determination will be made based on the student's ability to be in good academic standing as outlined in the Satisfactory Academic Progress policies by the end of the student's probation period (one semester block).

    • The student is responsible for submitting all relevant documentation, which will then be evaluated.
    • By not submitting all required documentation, the student's appeal will be denied for that semester block. In these cases, the student will have the opportunity to resubmit an appeal for a future semester block.
    • Students will be officially notified of the outcome in the my.maricopa.edu Student Center.
    • * No incomplete appeals will be evaluated for a determination.
  • Consortium Agreement

    Submit this form if you intend to take courses at two or more different colleges during the same semester/enrollment period and receive Federal Student Aid based on the combined enrollment at both institutions. Students on consortium who are not attending Rio Salado College as their primary institution are responsible for paying their account balance no later than the first day of class at Rio Salado College. 

    Deadline: The consortium agreement must be submitted by the start date of the class taken at the host institution. If there are multiple classes taken at a host institution, the deadline is the start date for EACH class.

  • FAFSA Parental Data Override Form

    Submit this form if you are a dependent student required to document the lack of parental data on your FAFSA. Students with approved submissions will qualify to be eligible to receive Unsubsidized Stafford Loans.

  • Federal Loan Discharged Due to Disability

    The Financial Aid Office has been notified that you have one or more prior student loans discharged due to total and permanent disability.  Submit this form in order to allow the Financial Aid Office to continue its evaluation of your request for Federal Student Aid.

  • FERPA Release

    FERPA, the Federal Education Rights and Privacy Act of 1974, is legislation designed to protect the release of student information. Without a Financial Aid FERPA Release on file, our office will not share any information about you to any other party unless we are subpoenaed to do so.

    • Submit this form with a third party in order to allow that party to access your financial aid record.
    • Before approval, verification of identity is required from both the student and the third party by submitting in person with state-issued identification, or by having the document notarized and mailed in with state-issued identification.
  • Homeless Independent Request

  • School Withdrawal Clearance Letter

    If you attended and received financial aid from another college during the 2012-2013 academic year, you must sign this form, and have your prior institution submit its official section. The information on this form determines whether or not you are eligible for aid based on your enrollment periods at the prior institution.

  • Request for Dependency Override

    Federal Law governing Federal Student Financial Aid assumes that the family has the primary responsibility for meeting the educational costs of students. The Rio Salado College Financial Aid Office may determine the dependency status for students who have documented, extreme, and extenuating circumstances where it would be unreasonable or impossible to provide the parent(s) information on the FAFSA. If you feel that your circumstances warrant re-evaluation of your dependency status, follow the steps below.

    • You may be asked for additional documentation depending on your individual situation.
  • Revision Form

    Submit this form to cancel your financial aid, request a student loan, and other miscellaneous requests. DEADLINE: For loan changes, this form must be received no later than 15 business days before your latest class’ end date. If you are no longer eligible for a loan, the request will not be processed.
