Proctor - Caremark

• All potential partnership proctors must complete a proctor application/verification form and receive authorization from their organization.
• A partnership proctor must be a current employee of the company.

Proctor Application Process

1. Complete the Proctor Application/Verification form (PDF).
2. Obtain appropriate authorization signature.
3. Mail the completed Proctor Application/Verification form to:

   Attn: Alicia Martindale
   2323 West 14th Street
   Tempe, AZ 85281
   
4. Upon receipt and approval, Rio Salado College will send the credit by examination process and all necessary information electronically to the proctor.
