Residency Forms

ID Authentication and Lawful Presence
All MCCCD students are required to provide a valid photo ID to enroll in courses. Those living in Arizona must provide proof of lawful presence in the United States for tuition purposes (eligibility for in-state tuition). Lawful presence is established by completing a Student Declaration Form and submitting appropriate documentation. All students who will be disbursed financial aid or who request a tuition refund will be required to validate their identification through this same process.

MCCCD Domicile Affidavit - Independent Student
To be classified as an in-state student for tuition purposes, a student must have resided in Arizona for a full 365 days prior to the start of the current term. Residency must be determined within the refund deadline of the first course of the current semester. Domicile Affidavits received after the refund deadline will be reviewed and processed for the upcoming semesters. Some visa types are not eligible for in-state residency, and a student’s status must be determined prior to the start of the semester. Ownership of property or payment of taxes within a state is not necessarily the sole basis for determining residency.

MCCCD Domicile Affidavit - Dependent Student (dependents of parent or spouse)
A one-year residency requirement is enforced to classify a student as an in-state student for tuition purposes.

MCCCD Domicile Affidavit - Military or Veteran Student
Our out-of-state students can receive the out-of-state military rate as long as they can provide documentation showing that they are either actively serving in the military, on Military orders, or LES (within the last two months). Veterans or students using a VA Edcuation Benefit can provide their certificate of eligibility. Do not make a copy of any military or dependent ID.

Federal VA Forms
- Apply for VA education benefits Form 22-1990. Students who applied for benefits before August 20, 2022, can submit a request online.
- Download your VA education letter if the student applied for Post-9/11 GI Bill (Chapter 33) benefits using VA.gov on or after August 20, 2022.
- Apply for education benefits as an eligible dependent. Students receiving education benefits under Chapter 35 Survivors' and Dependents' Education Assistance (DEA) or Chapter 33 Fry Scholarship can submit a request online.
- Apply to use transferred education benefits. Students using transferred entitlement benefits (TOE) can submit a request online.
